If you choose to join the SafeChurch site using your GuideOne policy information, you will need three items: 1.) your GuideOne policy number, 2.) the policy type, and 3.) your agent's five-digit number. This policy information can be obtained from your GuideOne billing statement.

The policy number and policy type are located under the premium statement section that is in the middle of the billing statement. If there is more than one policy number, you only need to enter one policy number and the corresponding policy type that appears to the right of the policy number.

The five-digit agent number is located in the upper right hand corner of the billing statement. Enter the agent number with no dash.
